The covered entity (CE), McCoy Vision Center, reported that its business associate (BA) experienced a hacking attack that compromised the protected health information (PHI) of 33,930 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnostic information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services to those affected.
