The covered entity (CE), Arizona Endocrinology Center, reported that a workforce member impermissibly accessed the electronic protected health information (ePHI) of 74,122 individuals. The ePHI involved included names, phone numbers, mailing addresses, and treatment information. The CE notified HHS, affected individuals, and the media.
