The covered entity (CE), RF EYE dba Cochise Eye and Laser, reported that it was the victim of a ransomware attack that affected the protected health information (PHI) of 64,710 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, local law enforcement, and posted substitute notice on its website. In response to the breach incident, the CE implemented additional technical safeguards.
