The covered entity (CE), Southern Connecticut Vascular Center reported that it experienced a cyberattack that affected the protected health information (PHI) of 154,417 individuals. The PHI involved included names, addresses, dates of birth, medical treatment information and health insurance information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its cybersecurity by implementing additional technical and security safeguards.
