The covered entity (CE), Aveanna Healthcare reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 65,482 individuals. The PHI involved included names, Social Security and drivers' license numbers, dates of birth, diagnoses, health insurance information, prescription information, and claims information. The CE notified HHS, affected individuals, the media, and posted substitute breach notice on its website. Following the breach, the CE sanctioned the employee, provided complimentary credit monitoring and implemented additional administrative and technical safeguards.
