The business associate (BA), Blue & Co., LLC, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 591,713 individuals. The PHI involved demographic, clinical, and financial information. The BA notified HHS and affected covered entities. In its mitigation efforts, the BA implemented additional administrative and technical security safeguards to better protect its PHI and retrained its workforce members on email security precautions.
