The covered entity (CE), General Health System, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 46,149 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, financial information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR provided the CE with technical assistance regarding the requirements under the HIPAA Security Rule.
