The covered entity (CE), Health Prime International, reported that it was the victim of a cybersecurity attack that affected the electronic protected health information (ePHI) of 17, 562 individuals. The ePHI involved included names, diagnoses, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards to protect ePHI.
