The covered entity (CE), Tidewater Diagnostic Imaging, reported that its business associate (BA) was the victim of a hacking attack affecting the protected health information (PHI) of 40,195 individuals. The PHI involved included names, Social Security numbers, dates of service, and other treatment information.
