The covered entity (CE), Wellfleet Group, reported that a student made the protected health information (PHI) of 23,082 individuals viewable via the Internet. The PHI involved included names, birthdates, addresses, diagnoses/conditions, and other demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
