The covered entity (CE), Monroe Ear Nose and Throat Associates, reported that it experienced a hacking attack that affected the protected health information (PHI) of approximately 14,500 individuals. The PHI involved included names, addresses, dates of birth, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
