The covered entity (CE), Wright & Fillipis, reported that it was the subject of a cybersecurity attack that compromised the protected health information (PHI) of 877,584 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and claims and financial information. In response to the breach, the CE provided administrative, technical, and security safeguards. OCR provided the CE with technical assistance.
