Pediatric Home Respiratory Services, LLC d/b/a Pediatric Home Service, the covered entity (CE), reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 746 individuals. The breached PHI included clinical and demographic information. The CE notified HHS, affected individuals, and the media. The CE took several corrective actions in response to the breach and OCR provided the CE with technical assistance regarding its health information privacy compliance obligations.
