Spurwink Services, the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 27,332 individuals. The PHI involved included names, birthdates, addresses, claims and health insurance information, Social Security numbers, diagnoses/conditions, and medication information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards.
