The covered entity (CE), Phelps County Regional Medical Center dba Phelps Health, reported that its business associate (BA) experienced a data security incident that affected the protected health information (PHI) of 12,602 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
