Benson Health, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 28,913 individuals. The PHI involved included names, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect sensitive data. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
