The covered entity (CE), Self Regional Healthcare, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 26,696 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
