The business associate (BA), MedQ Inc., reported that it experienced a ransomware incident that affected the protected health information (PHI) of 55,917 individuals. The PHI involved included names, addresses, dates of birth, drivers' license information, Social Security numbers, claims information, diagnoses, lab results, medication information, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring and implemented additional administrative and technical safeguards. Staff were retrained to better protect PHI.
