The covered entity (CE), Arlington Skin, reported that its business associate experienced a cybersecurity attack affecting the protected health information (PHI) of 17,468 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, lab results, and medications. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provide complimentary credit monitoring services to affected individuals.
