Fredericksburg Foot & Ankle Center, the covered entity (CE), reported that it experienced a cyber-attack that affected the protected health information (PHI) of 14,912 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers' license and state identification numbers, and clinical and health insurance information. The CE notified HHS, the affected individuals, and the media. Following the breach, the CE sold all of its assets and ceased all healthcare operations. Therefore, OCR closed its investigation.
