The covered entity (CE), Common Ground Healthcare Cooperative, reported that its business associate (BA) was the victim of a breach affecting the protected health information (PHI) of 133,714 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and diagnostic codes. The CE notified HHS and assisted the BA in notifying the media and affected individuals. Substitute notice was also provided. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative safeguards.
