The covered entity (CE), Total Vision, reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 138,402 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, and clinical and other treatment information. In response to the breach the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
