The covered entity (CE), Thomas Eye Group, reported that a misconfigured firewall and router resulted in a cybersecurity incident that compromised the protected health information (PHI) of 33,994 individuals. The PHI involved included names and addresses. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE implemented additional technical safeguards to better protect its PHI.
