The covered entity (CE), Hale Makua Health Services, reported a cybersecurity incident that compromised a desktop computer and a network server containing 14,369 individuals' protected health information (PHI). The PHI involved demographic, financial, and treatment information. The CE notified HHS, affected individuals, and the media of the breach, and posted substitute notice on its website. The CE also offered affected individuals complementary identity and protection services.
