Written by the Office for Civil Rights when the case was closed and reproduced verbatim from the HHS export.
North Ottawa Medical Group (NOMG), the covered entity discovered that its business associate (BA), Bizmatics had suffered a breach incident. The servers, which contained protected health information (PHI) were hacked. Bizmatics was aware of the intrusion in late 2015. Bizmatics informed NOMG that there was no evidence that NOMG's medical records were affected by the breach. However, at NOMG's request, Bizmatics submitted a breach report to OCR. NOMG submitted to OCR a copy of its Business Associate Agreement (BAA) with Bizmatics. This case was consolidated into an existing case.
