The business associate (BA), Navvis & Company, LLC, reported that it experienced a phishing and ransomware attack that compromised the protected health information (PHI) of 2,824,726 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses/conditions, and other treatment information. The BA HHS, affected individuals, the media, and provided substitute notice. The BA offered complimentary credit monitoring services to affected individuals and implemented additional administrative and technical safeguards to better protect PHI.
