The covered entity (CE), Outreach Health Services, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 60,414 individuals. The e PHI involved included names, Social Security numbers, drivers' license numbers, financial information, dates of birth, health insurance and clinical information, diagnoses, medications prescribed, and other treatment information. In response to the incident, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
