Sea Mar Community Health Centers, the covered entity (CE), experienced a breach that compromised the protected health information (PHI) of 688,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, identification numbers, X-ray images, and claims and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect PHI.
