La Red Health Center, the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 39,759 individuals. The PHI involved included names, dates of birth, addresses, clinical and financial information, and other identifiers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
